  You earn reward points every time a prescription is purchased using your Myperkpoints.com number!

  Simply select one of the following methods to distribute your personal Rx savings card:

  1. TEXT
  2. EMAIL
  3. PRINT
  4. ORDER CARDS

Points are accumulated and are redeemed for myPerkPoints Rewards.

  • Select your reward from Myperkpoints.com
  • Fill out the form and submit "CLAIM REWARD"
  • A Thank You "Your request has been sent" will confirm submission
  • You will then receive a confirmation email and text message with details on your reward
